On February 16, 2024, The vehicle (F150 XLT Super Crew) that was being driven was going approximately 40 miles ph when an other vehicle came out of a side street who had a traffic stop sign. There were two school buses blocking their view and my view of anyone coming out of the development. My vehicle hit head-on the other vehicle and I blacked out. No Airbag Driver side deployed, but the seat-belt tension-er fully pulled on me. I awoke with alarms going off and smoke in the cab and in the engine. Fire Department cut all wiring. I was taken out of the vehicle by Paramedics and taken to the hospital with fractured sternum and bruises. Police were on the seen and did a report with the other driver at fault. Insurance company proceeded to have the vehicle towed and inspected and totaled the vehicle. Did not have vehicle inspected by independent company that I know of. Police report stated no air bag deployment. Hospital report showed no air bag deployment. Issues a year earlier with engine replacement by dealership under manufacturer warranty due to engine failure. No other known issues appeared for the failure.

The contact owned a 2018 Ford F-150. The contact stated while driving 35-40 MPH, an oncoming driver veered into his lane. The contact stated that he swerved to the left to avoid a head-on collision with the other driver. The vehicle drove off the road into the woods. The contact stated that all the airbags but the front driver's airbag deployed. The contact stated that he was able to exit the vehicle through to front passenger's side door. Police officers assisted the contact out of the wooded area. The contact sustained a laceration between the right eye and the ear, and a laceration on the chin. Additionally, the contact sustained severe pain on the left hip and ankle. The contact did not receive medical assistance. A police report was filed. The vehicle was towed to a tow lot, where it was in the process of being totaled. The dealer and the manufacturer were not notified of the failure. The failure mileage was approximately 120,000.

On the morning of September 14, 2023 I was involved in a front end collision. I am concerned there was no airbag deployment. I was traveling 40mph 1 sec prior and slammed on brakes before impacting another vehicle. Vehicle total loss. Vehicle started flashing hazards and displayed a message on the dashscreen- I believe it had the word "safety" in the message. I was the sole occupant. I am 2nd owner of truck, which had clean vehicle history and no prior collisions. I have dashcam footage from my vehicle; person in vehicle behind me also provided dashcam footage. This is too large to upload but can be provided on request.

The contact owns a 2018 Ford F-150. The contact stated that prior to the crash he had driven into some mud and the ABS and engine lights were illuminated. The contact stated that while driving in snow at approximately 40 mph, the accelerator pedal became stuck. The brake pedal was applied but failed to respond. The driver made a sharp U-turn into a patch of grass and crashed into a fence, hydroplaned, and crashed into a dumpster. The contact sustained injuries to his back and neck that required medical attention. The passenger sustained unknown injuries and received medical attention. A police report was filed. There were no reported fires. The air bags did not deploy. The vehicle was towed to a collision center. The insurance company evaluated the vehicle and towed it to another repair shop. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The contact called the manufacturer who referred him to the dealer for diagnostic testing, but no additional assistance was provided. The failure mileage was approximately 83,465.

i WAS DRIVING AND SLID OFF THE ROAD AT A CORNER, WHEN I IMPACTED A TREE, THE AIRBAG DIDNT DEPLOY, ALTHOUGH THE VEHICLE IMMEDIATELY CALLED 911. CURERNTLY THE VEHICLE IS AT A CHEVY DEALER WAITING FOR INSURANCE TO FINALIZE PAPERWORK FOR A TOTAL LOSS. IT CAN BE INSPECTED IF REQUESTED. i CANNOT FIND SOMEONE TO LOOK AT THE FAILURE WITHOUT COST TO MYSELF. I FEEL THAT THE AIRBAG SHOULD HAVE DEPLOYED DUE TO THE FACT THAT THE BUMPER WAS SHOVED INTO THE ENGINE, AND THE FRONT OF THE VEHICLE WAS DESTROYED ON IMPACT. THE INSURANCE LOOKED AT VEHICLE, AND DID NOT FEEL THE AIRBAG DEPLOYMENT WAS WORTHY OF INVESTIGATING. THE POLICE DID NOT INSPECT ANYTHING. NO WARNING LAMPS OR SERVICE INDICATORS WERE LIT PRIOR TO IMPACT. I CONTACTED THE MANUFACTURER, AND THEY STATED ID HAVE TO TAKE THE VEHICLE TO A DEALER FOR DIAGNOSTIC. THE INSURANCE REFUSED TO SEND THE VEHICLE TO FORD. i HAVE NECK STRAIN AND PAIN AFTER THE ACCIDENT, AND FEEL THAT HAD IT DEPLOYED, I WOULD BE IN MUCH BETTER SHAPE. IF THE AIRBAG DEPLOYMENT IS SET TOO HIGH FOR IMPACTS, THEN THEY ARE USELESS TO PREVENT INJURY.

The steering wheel has cracked in the center area housing the AIR BAG and HORN. I believe this is a safety hazard. With the crack going over half of the way through the steering wheel, there is a chance of the AIR BAG DEPLOYING without cause.

The contact's husband owned a 2018 Ford F-150. The contact stated that while her husband was driving at 62 MPH, he hit the back of the semi-truck on the front driver which sent the vehicle to tailspin and caused her husband to hit the semi-truck for a second time. The second collision sent the vehicle into another tailspin before coming to a complete halt. A police report was filed. According to the police department investigation, the airbag inflator had deployed which struck her husband's forehead. The investigation also determined that her husband's injuries were not consistent with proper airbag deployment. The report also noticed that both the air bag and air bag inflator were found on the floor near the rear passenger side seat. In addition, the report showed bruising constant with the lower lap belt locking but the driver did not have any bruising/burns from the upper seatbelt. The contact was pronounced dead on the scene and was taken to a moratorium. The vehicle was taken to an independent tow yard and deemed a total loss by insurance. The dealer nor the manufacturer had yet to be notified of the failure. The failure mileage was 36,667.

I had a front end collision with my truck. The damage was a direct hit on the airbag sensor, and my airbags nor my seatbelts deployed. I was going about 45 to 50. The pre-collision warning appeared right before I hit the vehicle but no automatic braking. The body shop that has my vehicle had safety concerns because my vehicle took the brunt of the crash right on the airbag sensor. No airbags or anything deployed, but the sensor was broken in the accident. The concern is if the sensor was hit directly, why didn't it send a signal to through the system to deploy safety restraint systems. I called Ford to inform them of my concerns. After going through several people, I was finally given a case number CAS37213430 after days of trying to get them to take me seriously. They told me to take it to a Ford dealer to have it checked, but it is wrecked so how will they check it. My concern is that it didn't deploy the restraints because obviously wasn't working properly. I couldn't get all my pics to load so have more.

This vehicle was involved in an offset rear-end collision with a semi-trailer, with the driver's side making the primary impact. The crash resulted in the death of the driver and only occupant. Both the driver's airbag and left passenger curtain airbags deployed. During the police investigation of the crash, it was determined that the steering wheel-mounted airbag assembly completely dislodged from the steering wheel, with the airbag and its assembly being located in the rear floorboard post-impact. The driver sustained a head injury possibly consistent with the airbag assembly striking their forehead area. Two out of three of the plastic retaining tabs were found to have sheered off, likely leading to the airbag assembly being ejected from the steering wheel. Police investigators documented this incident with photographs and have taken the airbag assembly as evidence.

02/08/2022 Complaint Airbag steering wheel assembly when deployed detached like a rocket into my face and then blew out the rear window i am assuming or it was from the collision. Front end collision into the wall of the 101 southbound fwy in Mesa arizona all airbags deployed. i suffered air bag burns and bruising and my glasses frame were broken in half cutting my right eye lid. My concern is the steering wheel airbag deployed and the whole steering wheel airbag assembly detached from the steering wheel struck my face and possibly blew out the rear window. My investigation when steering wheel air bag deploys the airbag assembly remains attached to the steering wheel. i uploaded pictures of steering wheel airbag assembly notice the back was melted. in the collision the battery was damaged and post was severed possibly saving me from a electrical fire.

The steering wheel horn button, where the airbag is behind it, is cracked. This is the second time it-??s cracked since I-??ve on the vehicle. The first time it was fixed by Mike Castrucci Ford April 22, 2019. I actually spoke to someone at Kings Ford named Tricia just a couple of days ago, and she told me that it was not under warranty. I tried to explain to her that this has happened before when it was under warranty and fixed already, and this was the second time, but she refused to help me. It looks like it-??s called the AB module - airbag on this invoice I-??m holding. At times, it seems to leak some sort of liquid slightly from the cracks, and the horn does not always work when I push on the button itself. I have included a picture of the invoice when it was fixed once before, and I-??ll try to include pictures I took today of the horn button.

The contact owns a 2018 Ford F-150. The contact stated that while driving approximately 45 mph when the vehicle's side air bags on the driver-side, front and rear, suddenly deployed without impact. The contact then noticed that a small fire had sparked on the floorboard near their feet. The contact had pulled over to the side of the road where an observer was able to retrieve a fire extinguisher from their home and put out the fire. The contact was unable to verify if a fire report was made. There were no injuries. The vehicle was towed to the dealer who had not diagnosed or repaired the vehicle as of yet. The manufacturer was not made aware of the failure. The approximate failure mileage was 55,000.

The truck is less than 40,000 miles and the dealership states that the entire vista roof frame with cables has to be replaced because it will now not open. It makes a horrible noise and this was confirmed. Also I have had the same dealership check out the white residue that continues to show up on the cover of the steering wheel. They have replaced the cap, but the white residue continues to show up and now that it is out of warranty they will not fix it.

The air bags failed to deploy. A police report was filed.the contact sustained injuries and had broken glass in their forearm that required medical attention. The vehicle was not drivable and was towed to an impound lot. The vehicle was not diagnosed or repaired. A dealer was not contacted. The manufacturer had not been informed of failure. The failure mileage was 31,000.

The front dashboard warped up in front and on the passenger side over the airbag. It was reported to the dealership, Ford redd mccombs. Ford company approved to replace the dash. It was replaced after 5 months waiting. And no uncertainty of safety of dashboard over the passenger airbag, possibly deplying or exploding. The second dash replacement has warped in the same areas and more. The areas over the passenger air bag, as well as the driver side on front dashboard. The company as well as the dealership have no explanation as to why the dashboards are warping. I have seen and photographed many other trucks with same make and model and the suffer the same problem. If a person is not looking for the problem, they will never see it until it gets worse and the warrenty has expired and they are probably thinking its the Texas heat and sun. But this is a true vehicle hazard and will continue to be addressed further if you all don't wish to help or guide me into the right direction. Iys a flaw in the design and its in all F150 trucks from 2017 to 2020. I have seen the flaw in the 2020 model at the car show that was held in san antonio tx, back in November,2019.

Was driving back to my office and on the on ramp from a highway to interstate 95 South. I eased left to merge onto the interstate going about 45mph and increasing speed when my steering wheel pulled left quickly on its own power and steered us head on towards the metal guard rail in the center median. I tried to pull right but there was resistance. I noticed an 'uphil decent fault' on my dash just before my truck struck the guard rail. Luckily I was able to last second pull the vehicle right enough where it wasn't a full head on collision and my employee in the passenger side was just sore after the fact. I am currently undergoing medical tests for back, neck and shoulder numbness as well as tension headaches. No airbags deployed! Ford said once the body is fixed where they can run tests they will see what can be done, but I do not feel safe driving this vehicle. It literally tried to kill us. There was also a 'service advancetrack' light on the dash. I previously had those faults and the dealership said it was a wheel bearing that they needed to replace. Something is going on with these vehicles.
