Consumer was involved in a front end collision at 35 mph and seat belt did not work properly after slamming on the brakes, consumer was still able to be thrown forward, and hit his shoulders and chest. Contacted dealer, and dealer was going to take a look at the vehicle. Consumer also stated that the air bags failed to deploy.
The driver's seat appears to either 1) not be fastened securely to floor, or 2) have a faulty component which allows movement between the seat bottom and back rest. I have taken the truck to a nearby dealership five tmes. The repair work only provided a temporary fix. The problem is accompanied by a loud creaking noise similar to an old door on rusty hinges. Toyota's customer assistance center has assigned a case number 200009110317. They have not responded to my last written complaint.
