- the contact stated that while driving the 1999 Toyota Tacoma with 172000 failure mileage at 20 mph the vehicle hit a concrete pole on the center of the vehicle's bumper, and the air bags did not deploy. The driver sustained minor injuries. The contact stated that the vehicle was totally destroyed.
Truck was involved in a frontal collision at 50 mph. The driver nor the passenger side airbag deployed upon impact. The cause of the failure has yet to be determined.
While driving on I-5, just South of mt. Shasta, at about 2:45 in the morning, I hit a 400 lb brown bear, doing the speed limit for the road in the area(65mph). The bear brought the truck to a complete stop. Neither driver nor passenger side airbags deployed. I called a local Toyota dealer and they said that the bear was not a stationary object, and this is why the airbags did not deploy.
While stopping at a traffic light vehicle was rear ended at 80 mph, forced into another vehicle head-on. Upon impact, driver's and passenger's air bags did not deploy. The accident caused damage to the front end of the vehicle and injuries to the driver.
Vehicle was involved in a frontal collision of 40 mph with a 1996 GMC, Suburban. Upon impact, both air bags did not deploy. Dealer notified, and was unwilling to assist in this matter.

Consumer was traveling about 10mph, and in the process of turning left another driver was going straight, and both vehicles collided. Upon impact, airbags didn't deploy. Vehicle has dual airbags and no injuries.
Was traveling along the road at 40mph and rearended another vehicle that seemed to have been going at 5mph. At time of impact, neither air bag deployed. Driver was slightly injured, but had no hospital treatment.

While driving 35-40 mph the vehicle had a direct frontal impact in which the driver's and passenger's air bags did not deploy, which did not protect the occupant in this crash.
Consumer was driving approximately 30 mph. Upon impact, vehicle was struck at driver rear tire, vehicle flipped in air approximately 20 feet and landed on the front bumper, air bag did not deploy. Vehicle then flipped on its top and began to slide, and passenger's seat belt buckle came un latched.
