The contact owns a 2017 Toyota Sienna. While the vehicle was parked on an incline, the passenger side sliding door opened and failed to latch. As a result, the door retracted while the contact's arm was still inside the vehicle. The contact managed to jerk her arm out of the vehicle, but sustained bruises and scrapes to the arm. Medical attention was not received. There were no warning indicators illuminated. The contact took the vehicle to Toyota of North charlotte (13429 statesville rd, huntersville, nc 28078, (704) 875-9199) and a service appointment was scheduled for September 6, 2019. The vehicle was not diagnosed or repaired. The manufacturer was not contacted. The failure mileage was 45,397.

The contact owns a 2017 Toyota Sienna. While driving, the windshield resembled a magnifying glass approximately six to eight inches above the dashboard. The issue caused the driver to experience dizziness and headaches while driving. In addition, while driving approximately 65 mph, the rear driver side door opened without warning. The vehicle was taken to the dealer where it was diagnosed that the windshield had a third dimensional magnifying glass, which was a manufacturer's defect that could not be rectified. The vehicle was not repaired. The manufacturer was made aware of the failure. The approximate failure mileage was 1,800.
