Can smell strong gas fumes, after filling gas tank. Also, gas tank leaking, after putting gas in tank, at half fill or full tank, get gas smells and leaking. While stationary and moving, can smell gas fumes. Headlights do not provide adequate visibility at night and during rainy weather. Headlights adequate only in high beam mode.
The contact owned a 2012 Mazda 6. while driving 50 mph, the vehicle caught fire. The fire department extinguished the fire. A police report was filed. The vehicle was towed to an independent mechanic. The vehicle was included in NHTSA campaign number: 14V114000 (fuel system, gasoline). The manufacturer was not notified of the failure. The approximate failure mileage was 140,000. The vehicle was destroyed.
