The contact owns a 2010 Lexus LS460. The contact stated while driving at various speeds, a brake failure warning message appeared on the instrument panel. The contact stated that several unknown warning lights were illuminated. The vehicle was taken to wilkie Lexus (568 lancaster ave, haverford, pa 19041) where it was diagnosed with the brake actuator needing to be replaced. The vehicle was not yet repaired. The manufacturer was informed of the failure and offered to cover 20% of the parts cost. The failure mileage was approximately 121,000.

The contact owned a 2010 Lexus LS460. While driving 55 mph, the vehicle began to skid on the freeway. When the contact depressed the brake pedal, the vehicle failed to stop. As a result, the vehicle crashed into a side embankment, bounced back onto the freeway, turned 360 degrees, and crashed into the embankment again. The vehicle then bounced back a second time onto the freeway before it came to a full stop. Bystanders helped to coast the vehicle over to the side of the road. The air bags failed to deploy. The driver sustained neck and back injuries that did not require medical attention. The front passenger sustained a spinal fracture, eye socket fracture, and back injuries that required medical attention. The rear passenger sustained head injuries that did not require medical attention. A police report was filed. The vehicle was towed to the contact's residence. The vehicle was not taken to a dealer or an independent mechanic for diagnostic testing. The vehicle was destroyed. The manufacturer was not notified of the failure. The approximate failure mileage was 75,000.

I am experiencing an anti lock brakes actuator failure. Upon starting the vehicle it gives the warning "stop the vehicle and contact a dealer". I have did this and found that this is an issue that Toyota/Lexus is aware of as they have issued a service bulletin about it and offered an extended warranty. However, the extended warranty is limited by mileage and years. Being a key safety component in vehicle operation, I am requesting this issue be converted into a recall. I have tried to get repair assistance from Lexus customer support and they denied the request based on mileage.

The contact owns a 2010 Lexus LS460. While driving 65 mph, the steering wheel began to shake and vibrate when the contact depressed the brake pedal. Lexus of westminster in westminster, California diagnosed that the brakes were worn. The brakes were replaced, but the failure recurred. Auto clinic of irvine, California, a non-Lexus dealer, performed a diagnostic test and determined that the vehicle was included in tsb: L-sb-0036-15 rev, but the vehicle was not repaired. The manufacturer was made aware of the issue and stated that, although the vehicle was repaired once by a Lexus dealer, a Lexus diagnostic must be performed on the vehicle. The contact stated that the vehicle was under warranty for 100,000 miles until February 28, 2016 or until the mileage cap was reached. The failure mileage was approximately 40,000.
