After having our car in the local dealerships shop to fix recalls and a Very hard jerking of the car when slowing to 8mph thinking it was the ABS accutator, that unit was replaced and system bled and tested we picked the car up and before we got 100 feet away from the shop our car still had the same problem. After much talking with a lexus forum group I now believe based on their experiences and testing myself it's a Hard 2nd to 1st gear downshift ! and through my investigating, this is a common issue among lexus transmission between years 2006 - 2013

Automatic transmission. When slowing to a stop or anytime (everytime) the car needs to auto downshift from 2nd to 1st the car hard shifts and the entire car jerks really hard causing almost whiplash. I have a unsafe vehicle that is dangerous for myself and the public. The dealership has fail to take responsibility for the problem. There is a recall for the problem and it was extended as well. ( carbon buildup). The registration history on the vehicle is very strange, it does not seem to be accurate, I believe this vehicle was a defect and the dealership has tampered the vehicle to cover the history. The vehicle service history has been purge and deleted for unknown reason. I am getting different answers and no help with the problem. It has cause me a lot of distress and concern, I have spent thousands of dollars trying to make it safe but from research this is a common defect for this particular vehicle and it can not be repaired. The vehicle was taken to the dealer for diagnosis but they were unable to duplicate the problem. The vehicle was repaired. The manufacturer was not notified. The approximate failure mileage was 133,000.

Car parked parallel to curb. Opened door, got into driver's seat, fastened seat belt, turned on ignition, as normal. Shifted from park into drive, immediately engine began to "rev up" uncontrolled, based on sound from engine. After a few seconds of increasing pitch of engine noise, car jumped the curb, over the adjacent side walk, and slammed into the brick wall in front of my house, slid along the wall, and car ended up against the fence of next door neighbor. Car moved about 6 feet in 2-3 seconds. Right hand and wrist of driver was injured on impact, trapezium bone broken in half, and a portion of bone smashed into little pieces. Hand and wrist muscles and tendons strained. Surgery was required two weeks later to put 4 metal rods in wrist to assist healing. In cast for total of 8+ weeks. Many sessions of occupational therapy will be needed to restore wrist and hand function, some loss of function is anticipated, and arthritis in wrist may result. Car damage included front bumper, front grill, front spoiler, grill assembly, headlamp, head lamp assembly, fog lamps, rt front wheel, right front lower arm control, right, left outer tie rod.
