When removing my 4 day old infant (in her brand new car seat), I noticed the seat belt was very loose. I securely fastened her in before leaving to visit family for Easter. When I lifted the car seat, the seat belt buckle was laying in the base. The strap securing the seat buckle detached and came free. My daughter was not securely fastened while I was driving my vehicle!!!!
The contact owns a 2009 Honda Civic. While operating the vehicle, a loud popping sound was heard and smoke suddenly appeared from under the front passenger seat. It was later discovered that the front passenger seat belt would not operate and the air bag indicator illuminated. The vehicle was taken to the local dealer (motorcars Honda, 2953 mayfield rd., cleveland heights, oh) where it was diagnosed that the SRS malfunctioned and caused the passenger seat air bag to erroneously deploy. The vehicle was not repaired. The manufacturer was notified of the failure and did not assist. The failure mileage was 95,000.

After a passenger entered the vehicle and fastened the front passenger seat belt, the contact heard a loud noise and saw smoke emitted from the front passenger seat belt. There was no visible fire or burning odor. The fire department was not contacted. The vehicle was taken to Honda of fayetteville (1384 w showroom dr, fayetteville, ar 72704). A diagnostic test was not performed, but an unknown component was replaced. The manufacturer was not made aware of the failure. After starting the vehicle, the air bag indicator illuminated and another indicator illuminated, indicating that the passenger air bag was off. The vehicle was taken to Honda of fayetteville again where it was diagnosed that the SRS unit failed and needed to be replaced. The vehicle was not repaired. The manufacturer was not made aware of the failures. The approximate failure mileage was 17,000.
Takata recall. I was involved in an accident 1/10/17. My car was hit head on with a truck, 2001 GMC. The car was totaled and the report noted that the airbag never deployed.

The contact stated that the seat belt failed to restrain the driver and as a result, the driver hit the roof of the vehicle. The driver was not injured. A police report was filed.
