Front crash. Driver Seat pulled up and forward from base, contributing to crushing of driver. Air bags also did not work, so driver was crushed against the steering wheel (with no air bag protection). Death resulted. Medical examiners report lists injuries consistent with being crushed against the seat belt, and the steering wheel. GM needs to be held accountable. And, fix whatever failed with seat attachment.
