Vehicle accident resulting in one fatality. A witness stated the consumer's vehicle swerved immediately left of the center lane. The witness who was driving behind the consumer's vehicle, stated another van traveling in the oncoming direction tried to maneuver around the consumer's vehicle. As a result, the two vehicles met head on on the right side of the road. The other vehicle rode on top of the consumer's vehicle onto the roof. Both vehicles landed in a ditch. The consumer's husband (driver) was killed upon impact. Two children in the back seat were belted and sustained lacerations. Driver side air bag didn't deploy, the passenger side air bag did. Vehicle was heavily disabled and had to be towed. The driver had to be cut out the vehicle. The driver died as a result of blunt impacts to his trunk with multiple fractured ribs and lacerations of left lung, inferior vena cava. Liver and spleen. (wife on behalf on husband)
While traveling about 30 mph on the main road. She hit a bump inside of the road and her dual airbags deployed. The dealership is aware of the problem.

While going 40-45mph air bags did not deploy in a side collision. Vehicle jumped over a median and hit 3 signs. Also causing frontal damage. Dealer had been notified but had no record of the vehicle sold to this consumer. There was one injury.
Consumer states that the air bag light started flashing, in checking, found a short in the driver?S side air bag, took the vehicle to dealer they found a spring needed to be replaced and that repair would cost over $125. Dealer has declined repairs as they feel the auto is too old and has too many miles.

Vehicle was going 45mph and slid into a pole head on. Front end was totaled. Upon impact, front driver's and passenger's air bags didn't deploy, causing facial injury to driver. Dealer was notified.
Vehicle ran off road due to ice condition, causing both air bags to deploy with flames shooting out on both sides. Was able to extinguish fire before it became severe. Dealer and manufacturer were notified. Please feel free to porivide any further details on this matter.
A dear was hit on driver side, hood and front fender badly dented and need to be replaced. Air bags did not go off, and deputy on scene stated that they should've gone off. When repairs were made to the car the body man found no sensors on the car. At the time of accident the air bag light was flashing. But they never went off.
While driving at 15 mph vehicle had a frontal impact with another vehicle going about 30 mph, and driver's and passenger's air bags did not deploy, which did not protect the occupants in crash. No prior problems. Vehicle was totaled.
Vehicle was in a front end collision at 15 to 25 mph. Car was declared totaled by the american automobile association (aaa). Also, air bags did not deploy. Dealer has been contacted.
Driver's foot slipped off brakes onto accelerator, causing vehicle to run into 4-5 inch wide steel pole which resulted in front end being pulled back about 12 inches. Dual airbags did not deploy and seat belts did not catch, causing head injuries.
I am not sure how to contact you. I am a trauma surgeon and just admitted a four year old who was in front passenger seat with seat belt on and mom hit a deer deploying the airbag.
