FOURTH INCIDENT: On [XXX] 2024, at approximately [XXX], I was traveling about 40-mph and slowing for a stop light. Just before coming to a complete stop, at about 5-mph this vehicle suddenly shifted into park on its own. At the same time numerous & various warning prompts appeared on the screen and ALL the warning signs on the instrument cluster became illuminated. As with the three (3) previous incidents, after shifting into drive and taking my foot off the brake the vehicle accelerated all on its own without my placing foot on gas pedal. Unlike previous times, additional warning prompts came up on the screen, e.g. "Hill Assist Parking Unavailable" and "Auto Hold System Fault". Also, after parking, turning vehicle off, locking doors and waiting 30+ minutes, the vehicle did not reset itself like before. Previously after sitting and not running when started it operated properly but with just the engine light on. On May 26, 2024, I had a scheduled recall for the brake lights. I asked my dealer (AutoNation Ford, Frisco (Texas)) to look at video I had taken and a possible fix under Recall 24S33, which I had found on the internet. The dealer service rep refused to look at anything and received to service this particular complaint. Instead, they sent me back onto the highway with a vehicle that can suddenly lose power and accelerate on its own, thereby creating a risk of a crash and injury or death to others or myself. 1st time [XXX], Complaint #11583215 2nd time [XXX], Complaint #11590922 3rd time [XXX], Complaint #11592069 4th, or current incident [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Without warning the truck will stop itself and the parking brake will engage. The service engine light will come one along with the service power train malfunction lights and the pre collision assist not available light. When the truck has been put back into gear it will accelerate on its own but only up to 35 MPH no matter how much gas is applied. This is the second time this has happened both were identical. After sitting overnight with the truck off the issue goes away temporarily but will come back with no warning. Vehicle may be inspected. Ford has already looked at the issue once and is scheduled to look at it again. If this had happened while on the highway, I could have been in a serious wreck that could have been fatal.

THIRD REPORT SAME PROBLEM: See NHTSA tracking Number 11583215 & Complaint Number 11590922: On [XXX], I was driving 2-4 mph through an apartment complex drive when vehicle came to a sudden and complete stop and switched into park all on its own. Numerous & various instrument cluster warning lights and prompts were going off. After putting back into drive very little power but would accelerate all on its own up to 35-40mph. I had previously taken this vehicle on [XXX], to the dealer (AutoNation Ford, Frisco, Texas) and they kept vehicle four (4) days. Upon return they told me diagnostics showed no alerts on vehicle, thus no problems. So, nothing was done. A second such incident happened on [XXX]. The next day I again took this vehicle to AutoNation to have the previously scheduled brake light Recall fix. At same time asked them to again look at the vehicle for issues in this complaint. I also offered to show them video of the self-acceleration, warning lights & prompts and a possible Recall #24S33 (powertrain control module) for this issue. Autonation rep said I was there only for scheduled recall and refused to look at videos or other information. Vehicle WAS NOT examined. On [XXX], I was driving 2-4 mph through an apartment complex drive when vehicle came to a sudden and complete stop and switched into park all on its own. Numerous & various instrument cluster warning lights and prompts were going off. After putting back into drive very little power but would accelerate all on its own up to 35-40mph. This vehicle has had four (4) recalls, and now three (3) separate incidents as described above. I believe it is a hazard and the issue complained of now and previously, are of such magnitude that they pose a significant risk of accident potentially causing injury or death to others or myself. SECOND REPORT SAME PROBLEM: See NHTSA tracking Number 11583215, 04/16/2024. Took to dealer (Autonation Ford, Frisco, Texas) and they kept vehicle four (4) days. Upon return they told me diagnostics showed no alerts on vehicle, thus no problems. So, nothing was done. 05/26/2024 approx. 8:05pm, traveling 50-mph had sudden loss of power and speed. Warning lights and prompts appeared on cluster. Vehicle slowed immediately to 35-40mph and would not accelerate any faster. Sudden slowdown caused vehicle behind (who was following to close) to suddenly brake, almost hitting me. Would slow when brakes applied. When brakes released vehicle would accelerate to 35-40 mph on its own without applying gas pedal. This vehicle has had four (4) recalls, and now two separate incidents as described above. I believe it is a hazard and the issue complained of herein, and previously, are of such magnitude that they pose a significant risk of accident potentially causing injury or death to others or myself.

On Friday, May 17th around 2:00pm I was driving approximately 25mph when I got an abrupt warning and the truck started to drive differently. I don-??t think the vehicle was responding to the gas pedal normally. The first warning said something about selected gear engaging at lower speed. Then there was a message about the pre-collision assist not working, hill ascent not working, and something else - reduced powertrain/service engine soon/advancetrac or something like that. As I pulled over and was slowing down, the truck abruptly threw itself into park while going 5-10mph. I turned it off and then back on and put it in drive. The truck began accelerating without my foot on the gas pedal. My truck got all the way up past 20mph before I slowed down. I tried to record this with my phone as there was no traffic. I could not see well what I was filming because I was looking at the road. My safety was put at risk because had this happened on the highway or a road with no shoulder, things could have been a lot more dangerous. It was scary enough going at a lower speed on a street without traffic. I noticed after this happened that I had a third open recall for the powertrain control module. If I would have known I had this recall and how serious it was, I would have brought it in immediately. Ford should have done more to ensure vehicles impacted by this recall were contacted ASAP. I brought my truck in to the Ford dealership first thing this morning and I am waiting to hear back. The service agent was aware of this -??limp mode-?? situation occurring. I believe I was the first Maverick owner to bring a truck in for this issue at my dealership. There were no warnings that this might occur. I went from driving leisurely to having to figure out what all the beeping and flashing alerts were and manage to get to the side of the road in a truck that was not driving/responding normally. Then the truck started accelerating without my foot on the pedal! I have a video of this

[XXX] On [XXX] my wife and were traveling on [XXX] at interstate speeds and after traveling for about 55 minutes with no issues. The vehicle suddenly lost power and a lot of warning lights came on. We were unable to accelerate and quickly pulled to the shoulder. [XXX] is very busy and dangerous. It would not move forward while in drive and pushed on accelerator it would not accelerate. We called with Ford Pass for a tow. They towed it to Dickson TN Ford dealership and it is still there. They were to call us once they found out the issue. Been there now 6 days no call as yet from them. Very frustrating, vehicle with 16 K miles and only had a bit over a year and now more issues. 11 May 2024, I had started my truck as normal and began driving. About 2 miles from my house I lost all power to the truck. I was driving about 35 MPH when the truck threw up errors on the screen and I could no longer accelerate. It threw on the E Brake abruptly. Luckily no one was behind me as such an abrupt stop could cause a deadly accident. After I shut off the truck and waited 5 minutes and started the truck back up but it had a check engine light. I follow a forum called -??ford maverick truck club-?? where I was made aware of this issue happening to maverick owners after a recall for the tail light. I had this recall preformed. I drove to a fort dealership where they attempted to fix the issue but said there was no remedy and only cleared the codes. I tried to contact ford by chat support to make them aware but they were not interested in me reporting any potential safety problems.

Vehicle's engine suddenly cut out during driving ~15 mph. Multiple warning lights flashed. Vehicle ran on battery and was able to go at slow velocity but then transitioned to accelerating when pedal was not engaged. Hazard lights would shut off suddenly after driver turned them on. "Limp mode" is commonly used term for what happened.

Approximately around [XXX] a.m, [XXX], I was on my way to on the [XXX]. I was going driving around 75-78 mph when all of sudden my 2023 Maverick drop in speed and power suddenly. I looked at my dashboard and saw mph dropped to around 50. I had no knowledge of what was taking place at the time. I tried to press my acceleration pedal and the mph still was dropping, so I managed to get over to the far right lane so I could get to the shoulder. During this event several warning lights flashed, such as check engine soon, crash assist not available, power train malfunction ect. I turned the vehicle off and back on - it still had the same messages- I managed to drive the 5-10miles I had left on my route to work @ 33mph. Once at safe location I contacted a tow truck to haul my vehicle to Hennessey Ford for diagnostic and answers. I've never had any of these issues to take place since owning my truck. Never experienced Loss of power ever in my life. This is truly a event that has left me stunned and wondering will this truly be taken care of and addressed. It's a constant reminder in my head and and it also has prompted me to thank God for my life as well as others because if I hadn't left home to go to work about 30mins earlier than normally due to the traffic which would be pretty thick I may not be here today to tell this story, because there was no warning whatsoever. While driving, the vehicle will suddenly flash a message about gear selection being unavailable until speed is reduced, followed by error messages about collision-assist disabled, ESC disabled, hill-start assist disabled, and service messages. Check engine light and service lights also come on. No response to throttle inputs, once vehicle decelerates to around 1-2 mph electronic parking brake is automatically applied leading to an abrupt stop. Parking brake can then be removed and vehicle put back into drive, however all drive mode selection is disabled and again there is no response to throttle inputs - truck will accelerate on its own up to above 30 mph (do not know top speed as all areas this occurred were 30mph or less, had to ride brakes to maintain speed limit as vehicle was attempting an uncontrolled/uncommanded acceleration). This has happened 3 times, and first occurrence happened after recalls 23E07, 23E09, and 23C41 were completed. Any attempts to clear faults or exit this limp mode condition fail unless the vehicle is turned off and allowed to sit for 5-10 minutes, at which point faults still remain in system however vehicle is no longer in limp mode with functionality now restored to previously disabled systems. Recorded faults from last occurrence on April 05 2024: SOBDMC DTC U1011:86-AF SOBDMC DTC U1028:00-26 PCM DTC U1010:00-AF OBDII DTC U1011-P ABS DTC U0401:00-2F ABS DTC U0131:00-2E ACM DTC U0255:00-28 IPMA DTC U0418:86-AF In my situation all 3 failure events occurred in a low-traffic, low-threat environment. 1st happened when turning onto a 30 mph road and accelerating in low traffic, able to safely pull over. 2nd happened when turning out of my driveway and began to accelerate, again able to safely pull over. 3rd happened when leaving spot in parking lot, able to safely pull over into another spot.

While Driving on I-79 on Friday April 12th around 3pm my Ford Maverick would not accelerate when I pushed the gas pedal. I attempted to pass a tanker truck and as I pushed the gas pedal nothing happened. No acceleration whatsoever. As I looked down at the instrument panel I do not recall if any dashboard light lit up. I quickly pulled over on to the left hand shoulder of the interstate as I had an SUV barreling down on me from behind during the rain storm. I turned the engine off and waited about a minute before restarting the engine. It worked fine after that. I had it on the interstate since then and had no issues with it. I am on a Ford Maverick message board and many other owners have had similar problems.

I had just driven roughly 30 freeway miles on my way home and the truck suddenly flashes a barrage of errors, ESC fails, gear selection at low speed message, hill assist disable, pre-collision assist disabled, ABS light is on, MIL is on, service wrench is on, and drive mode selection is disabled. My truck lost all response to gas pedal, slows down and then slams on the parking brake at like 1-2 mph. I shut it down, started it back up. I had no gas pedal input. The truck self-accelerates up to 30-35 mph. I don't know at which speed it would've stopped accelerating because because the speed limit was only 30 mph. I ended up just riding the brakes to keep the speed down for 3/4 mile so I could get home. I went to reverse into my space at home and the truck was self accelerating in reverse as well. Normally it just rolls back slowly, but this truck was really moving. I abandoned the idea of backing in because I was scared it would just drive itself into the wall. I will not go back out on the roads in Los Angeles driving this vehicle until this is fixed, I would knowingly be putting my life and others lives in serious danger. I was fortunate that I had read about this happening in the past few weeks to several other Maverick owners who have had this problem, so I was aware of what was happening when it started and was only going about 30 mph and there wasn't much traffic around because it was a rainy night. This would've been horrifying if I were on the freeway or in moving traffic, which has happened to others. According to what I've found on the internet, this issue it is a result of some bad software installed in a recent recall. I had the recall done on [XXX] when I had an oil change. Not sure if this is appropriate but this is a reddit thread from a month ago with a bunch of other drivers that have had this happen: [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

When driving down the road at approximately 35-40mph, the accelerator pedal stopped working and a variety of warning messages and lights popped up on the dash. Some of those messages included "gear selected will engage at low speed", "powertrain system fault", and "pre-collision assist not available". Because the vehicle suddenly became undriveable, it rolled to a stop in the middle of the road. After coming to a stop, I put it in park (I may have restarted the vehicle, but am not entirely sure) and I was able to get the vehicle off the road, albeit with reduced power. Warning messages and lights (check engine, wrench) remained on the dashboard. There were no warning signs prior to this happening The vehicle was towed to a local Ford dealership and has been there for 13 days. I have not heard from them about identifying the problem. The uploaded photo is a screenshot on my phone of the warning messages through my FordPass app.

A couple of weeks after getting some software (or maybe firmware?) recalls done on my Ford Maverick my husband and I were driving down a semi-residential street. Suddenly the car went into limp mode and flashed a bunch of lights on the dash. So the car was limping along, but when my husband took his foot off the brake, the car started accelerating on its own. The car would brake when he hit the brakes but the second he lifted his foot off the brakes the car would start accelerating to about 40 mph. We were only a couple miles from our house, and we were able to get home safely, semi-careening our way there. Luckily there weren't any kids playing in the street (there often are) or any pedestrians crossing somewhere unexpectedly. The next day, my husband took the truck to the Ford dealership and told them about what had happened. He told them about how it went into limp mode and then accelerated by itself. They had the truck for basically the whole work day, and then brought it back. They said they couldn't recreate that failure so they cleared the codes, and told us to bring it in if it happened to them. When we got the documentation from the service it said that all they did was clear the codes. And they did NOT document ANYWHERE that we had reported that it had accelerated on its own. That night we found a Reddit thread that had lots of other people reporting the same failure and no satisfactory explanation was given, and, like us, none of them was certain that the issue had been "fixed." I'll upload the documentation they gave us, but again it's what they did NOT document that caused the most concern.

On the middle of the road when driving, of a sudden, powertrain malfunction, pre-collision not available message pop up and the service light, check engine light also started blinking on the dashboard. Gas pedal was unresponsive and the truck coasted, was not able to drive, and I was luckily able to merge over into the shoulder. Started the truck a few times and the issue persisted. Could move forward with the battery but still no input from the gas pedal. I towed the truck and is with the dealer. But i am really scared to drive it again. I was lucky to be on the slow speed zone road and was able to get out to the shoulder. However, I might have been hit and killed if it happens in the highways. Very scary moment. Ford should take some safety responsibility more carefully.

I was driving on a divided secondary road and my truck when into reduced power mode, displaying " Powertrain malfunction/Reduced power, engine service soon, and precollision assistance not available". This ended up slowing me rapidly from 55 down to under 30 +/- MPH. When it was necessary to stop at a light and I took my foot off the accelerator, the truck did not immediately slow but actually surged forward. After 'crawling' home, I had it flat-bedded to a dealership that day, and it took them almost 2 weeks and multiple test drives to tell me that that cannot recreate this issue. I was told by the service advisor that there was an issue and that could see it in the vehicles history. I went to the dealership and asked them how long they would keep testing it if they were not able to replicate the problems. They said 'it was up to me'. I took the vehicle home. We only had it for a day and less than 100 miles of driving and the problem occurred again, after dark, on a busy road in an intersection and the truck would not move at all for a few tense moments. The gear selector dial flashed repeatedly and the parking brake came on by itself. We had the truck flat-bedded back to the dealership the next day and it will stay there until the solution is found to eliminate this dangerous and life-threatening operational fault. The second incident occurred on March 26. Service to address 2 recalls was done on the vehicle on March 6.

Upon exiting highway and getting on access road, vehicle malfunctioned and indicated to slow down and shift transmission into neutral. I lost speed and was given little if any power to accelerate. I arrived at a red light where I shifted into neutral and then park. I shifted back to drive where I was able to get very little acceleration. I turned right, put on my hazards, and looked to park as quickly as possible. Even though I was driving slow I was also unable to fully brake, which was unsettling. I found a parking lot just a little ways up the road where I parked and called for a tow truck. The truck is currently at a local Ford dealer.

Was traveling at around 60 mph and multiple lights and warnings came up on the dash -??powertrain malfunction-?? and -??service advancetrac-??. Power was reduced to a limp mode and I was forced to pull over on the highway. This happened within a week of having multiple software recalls carried out by a ford mobile technician. I was able to reset the codes with an obd scanner and haven-??t had an issue since but I have read about this happening to multiple people with the same hybrid vehicle after having these recalls done.
