The tensionier pully assembly failed (bolt snapped allowing pulley to come free and housing to rotate out, binding fan and idler pulley. If at high speed, damage could have been substantial. I do not feel like this type of failure is reasonable with this number of miles, and the risk factor of possible accident or injury if the failure had led to the fan disintegrating, or internal engine damage due to stress and strain.
