The contact owns a 2005 Ford F-150. The contact stated that the seat belt failed to lock into it's buckle. The buckle fails to activate the lock and release the seat belt. The contact stated that while driving approximately 45 mph when a vehicle slipped on ice in front of their vehicle, in order to avoid crashing, the contact ended up steering into a nearby ditch crashing into trees. The seat belt buckle failed to secure the seat belt causing it to swing and hit the contact. The air bags did not deploy. The vehicle was not towed. A police report was not filed. The contact stated that the seat belt caused his teeth to be fractured and medical attention was needed. The vehicle was taken to a certified mechanic who had not diagnosed anything regarding the seat belt as of yet. The manufacturer was not made aware of the failure. The approximate failure mileage was 200,00.

On 9-14-19 I got into my truck to drive. Without realizing it, my seat belt did not recoil the last time I got out of my truck. When I reached to put the seat belt on, without knowing it, the seat belt wrapped itself around the seat reclining handle. While driving I stopped at a light and the belt must of tightened up more and pulled the reclining handle which put me in a lateral position while driving. If I hadn't pull myself up by the steering wheel and slam on my brakes, I would have hit the vehicle in front of me! I am afraid to drive my truck anymore without checking on the seat belt every day.

Yes a drunk driver ran into me and I think the officer let him just call his wife to come get him cause he was to drunk to drive and it was maybe involving him doing on repairs to the officers car the reason he got off scot free, I may could understand it but he like broad sided me and sent me flying across the seat and I saw him take off and so I was reaching around the steering wheel to try and make a sharper turn and give it some more gas evidently it did not work cause this was June 2 of 2018 and my shoulder is still hurt, he has insurance I have insurance but after the emergency room visit I have not been able to hire a lawyer, I have not even been able to get a doctor to tell me what's wrong with my shoulder, I can't get into an ortho group cause they say we don't take third party checks, I finally got into the regional one in memphis and made the appointment 4 months ago, and still have a month to go, it is not fair, I have contacted the attorney general of Mississippi and Washington, I have sent emails to the senator the governor and they referred back to my local representatives which I haven't heard back from them and that was a couple months ago, I only get about 3 hours sleep at a time and have been living off my savings for the last 8 months, it is almost gone and when I do get in I am not sure if I will even have the money by then to pay for the gas to get back and forth during the procedure of getting my shoulder fixed, I think you need to use the money from buzzed driving is drunk driving and use it to help the innocent people that have been injured in a wreck by a drunk driver especially if the officers don't I force the law, because when there is no law enforcement there is no law in my eyes, my name is [xxx] and I live in [xxx] and I have a lot more to add to this story but evidently don't have enough space, please help information redacted pursuant to the Freedom Of Information Act (FOIA), 5 U.S.C. 552(B)(6).

Vehicle is an 05 Ford F150 FX4 crew cab. Driver's seatbelt unlatching while driving. It happens about every other day of driving everyday. Sometimes it happens on a sudden brake, bumps, or sudden acceleration. Something in the seatbelt latch is not holding the buckle good. If a crash occurs, driver will be ejected from the vehicle eventhough driver is buckled up.

My truck has 56K on it bought it used with 35K on it when it starts runs with high idle and the top end ticks really bad the ticking does not go away at all speeds at a stop light it sounds like a diesel tic tic tic sounds like a lifter is stuck. I put "lucas" in it to try to make it go away then I changed the oil ran about two weeks with no difference and changed it again and no change called Ford dealer they couldn't help they said I would have take it in and get charged $165. Then I changed the spark plugs maybe a fouled plug that wasn't it there goes 120 bucks a couple of weeks ago my wife was driving on the freeway and the drivers side window falls the window regulator broke $140 replaced that already. The passenger seat belt locks up sometimes now to the front the steering wheel shakes really bad when you apply the brakes turned the rotors at Napa $20 new brake pads $90 at kragens balanced tires free I work at a tire shop shaking went away for about three days 350 miles later the shaking is back with a vengeance checked the balancing on the tires they were good rotors are out of shape again still on there now and the shaking is getting worse I cant afford them right now so they will stay there hooked up the snap on ehtos computer to it and registers that two sensors have an open circuit but no explanation for top end noise injectors might be bad.
