The right rear tire came apart causing the vehicle to veer to the left spinning around onto the median and struck the concrete barrier with the right front, then continued to spin and struck the barrier with the right rear, the vehicle then overturned several times and came to rest, one person was killed and the other suffered non-incapacitating injuries (Florida traffic crash report Bridgestone/Firestone P235/75R15 wilderness M/S W2hl1py416 right rear). Upon purchase of vehicle, there were Firestone Wilderness AT tires (no other specific tire information mentioned) that were original equipment on vehicle, consumer replaced with 4 new tires, however, after replacement, Firestone Wilderness AT tires were recalled, conumer submitted refund form, but was denied reimbursement.
Consumer was told that all four Firestone Wilderness AT tires (P235/75R15) had a cut line appearing completely around each tire and that they were capable of tread separation, only the two front tires were under recall 00T005, the two rear tires were in the same shape (including spare), but did not fall under the recall, consumer wants to be reimbursed for tires not under recall.
The tire blewout causing loss of control of the vehicle and an accident, as a result 4 people were injured (tire size 235/75R15) (claim # 43-R479-516). Nlm

In contacting Firestone to receive a rebate form for reimbursement of the replacement of the wilderness tire that blow out for me, the firsestone 800 number and the local Firestone dealer does not have and will not provide a copy of the reimbursement form. It nice that there is a recall for the 16 inch tires, but a customer cannot receive the form from either the manifacturer or their corporate retail outlet. (tiresize: 16)
Three Firestone wilderness ht tire failures. Consumer was traveling 70 mph on dry road conditons when front drivers side tire went flat without warning. Second tire failure was on front passengers side. Dealer has been contacted. Consumer stated he is unable to get any assistance from Firestone or dealer, consumer has now replaced all 4 tires.

Tread separation - tires replaced by dealer mike davidson Ford, jacksonville, fl. Firestone contacted - stated that tire is not included in recall - only subject to standard warranty; requested that tire that failed be shipped to Firestone, norcross, ga for inspection. With this tire failure, I had 4 tires replaced with another brand as a safety issue. (dot number: W2uuipo237 tiresize: P225-70R15)
While driving, Firestone wilderness tire became flat, a local Firestone dealer stated that a somewhat angular basalt rock had penetrated the tread and caused the tire to go flat, dealer also stated that vehicle was not designed to be driven off of paved roads, consumer feels this happened because the tires have very thin treads and sidewalls, a couple months after the first tire failure another tire failed exactly the same way, consumer had the front two tires replaced after the Firestone dealer looked at the numbers on the tires.
