While I was driving, the passenger seat headrest exploded forward. Chrysler calls this the "Front Seat Active Head Restraint." This is a safety mechanism supposed to be triggered in a crash, but the plastic holding back the force of the strong springs gave out and the plastic broke. This is a known issue that has caused others concussions, neck injuries, etc. As of yet the person who was sitting in the seat has not shown adverse symptoms, but his safety was put at risk. Chrysler is aware of this issue and had sent out a letter extending the warranty to 10 years, but this vehicle is just past the 10-year mark. No dealer, police, or insurance representatives have inspected this specific incident. There were no warning lamps, messages, or other symptoms of the problem prior to failure.

The contact owns a 2013 Dodge Grand Caravan. The contact stated while driving approximately 65 MPH, the ignition turned off and the vehicle coasted. The contact stated that power steering and the brakes failed to function. The contact coasted to a parking lot and was able to restart the vehicle after several attempts. The contact stated that the failure had become a recurring failure. The contact had taken the vehicle to an independent mechanic who diagnosed that the ignition switch needed to be replaced. The vehicle was not repaired. The manufacturer had been informed of the failure. The contact researched online and related the failure to NHTSA Campaign Number: 14V373000 (Electrical System, Air Bags). The failure mileage was approximately 160,000.

The contact owned a 2013 Dodge Grand Caravan. The contact stated while driving at 25-35 MPH with someone in the front passenger seat a semi-truck crashed into the rear of the vehicle. The vehicle spun seven times and continuously hit the concrete barriers on each side of the highway. The contact stated that the body of the vehicle shifted 5 inches away from the frame. The air bags were deployed but were deflated. The contact became unconscious temporarily and upon regaining consciousness, he noticed that he was laying flat in the front driver's seat as the seat had fractured into two. The contact also noticed that the driver's seatbelt had snapped. The contact was able to exit the vehicle with the assistance of emergency units and the fire department. The contact was transported to the hospital by the emergency unit. The contact sustained consistent middle back pain that would sometimes cause him to crawl instead of walking, numbness in both legs, consistent headaches, and Impaired vision. The other passenger sustained a seatbelt mark and trauma. A police report was filed. The vehicle was towed to a tow yard where it was totaled. The contact referenced NHTSA Campaign Number: 13V291000 (Air Bags). The dealer and the manufacturer were not notified of the failure. The failure mileage was approximately 69,000.

The vehicle was damaged and towed to a tow yard. The vehicle was not diagnosed or repaired. The dealer nor the manufacturer was not notified of the failure. The failure mileage was 68,000.

On 12/10/22 the passenger side curtain and squib airbags deployed with no collision of any kind. I was picking up my disabled partner, who is under 5' and under 90 lb. I believe she would have been at significant risk of harm were she already in the van. On 12/12/22, I took the van to where we purchased it. The van is BraunAbility modified with an accessible ramp, the back seats removed, and it sits much lower to the ground than the unmodified van. First, I was told that they found evidence of a collision. I had long known of that damage: I had ran over a curb several years before, and, because of the modifications, the van tends to scrape on acute inclines, a problem they said at the time of purchase would sounds scary but was harmless. I was told that the tiny hole in the metal frame under the van "exposed the sensor" and that deployed the airbags. I knew this false, but since that indicates a defect, I wanted it explicitly on their report. Instead, a manager came out and said she had completely misunderstood what the BraunAbility rep had said. He calmed that riding up a curb was sufficient to deploy the curtain and thorax airbags, but conceded that they couldn't say when the rusted-over damage occurred. Still, I did not run over a curb: I was in the left lane of a two-lane, one-way road and the old damage was on the passenger's side. They suggested filing an insurance claim, but that would put others at risk if the defect isn't limited to our van. There were no warning lights, messages, discernible symptom before the failure. I'm uncertain how to proceed. Should I go to a Chrysler dealership?

The following has happened to my car. The ahr headrest went off, and I wasn't in an accident. However, Dodge Chrysler is not wanting to issue a recall since a person is not at risk for injury. However, cost to fix this headrest runs in the $500-$900 category. And it's a known issue. So why isn't Dodge Chrysler acknowledging it? the 'active head restraint' (ahr) headrests found in certain Chrysler vehicles and manufactured by grammer ag suffer from a defect. The 65-page lawsuit out of Florida alleges that although fca us and grammer ag's active head restraint is meant to spring forward in the event of a rear-end collision to catch a vehicle occupant's head to prevent whiplash, the feature can 'deploy without warning or external force from a collision, and forcefully strike the back' of a driver or passenger's head. Describing the apparent defect as common and uniform, the case claims the force of the impact to the back of a vehicle occupant's head and neck can cause serious injury and creates an additional accident risk should the defect manifest suddenly while a vehicle is in operation. According to the lawsuit, the cause of the defect is the failure of a 'cheap plastic component' inside the defendants' ahr. Specifically, the feature contains a plastic bracket that acts as the triggering mechanism and holds the feature's spring-loaded release in place until a sensor signals that the vehicle has been hit in its rear, the case explains. As a 'cost-saving measure, ' the plaintiffs say, Chrysler and grammer ag designed the ahr bracket with 'an inferior and inexpensive form of plastic' that can crack and break down prematurely due to the 75 pounds of constant pressure applied by the ahr's springs onto the bracket.

2013 Dodge Caravan driver side head test deployed for no reason while stopped at a red light on the morning of September 8th, 2019. My vehicle was stationary. No accident or rear collision occurred. Photos of front and back of vehicle will prove no damage. It appears the plastic retainer which holds the component together failed and snapped on its own Accord. This is a safety issue and needs to be addressed by Dodge/Chrysler.

The contact stated that while driving approximately 10 mph, another vehicle failed to obey a red light at 35 mph and crashed into the front of the contacts vehicle. During the crash the front end of the vehicle was severely damaged but the air bags did not deploy. The driver sustained a right hip and an ankle injury along with a concussion. The vehicle was destroyed and towed away. A police report was taken at the scene. The manufacturer was contacted and an inspection was performed on the vehicle but no problem was found. The local dealer was notified. The contact mentioned that there was a recall related to the air bags associated with her vehicle but the recall information was not available. The failure mileage was 190,000. Consumer stated airbag recall in my vehicle due to defective software being installed at the factory, which would cause airbag failure in auto accident. Consumer would like to request a vehicle examination and review of an airbag system failure, in an auto accident that totaled my 2013 Dodge Grand Caravan. 'parts of this document have been redacted to protect personally identifiable information pursuant to the Freedom Of Information Act (FOIA), 5 U.S.C. 552(B)(6).'

I have installed a blue driver diagnostics tool to my vehicle and reported that I have a recall for my vehicles airbag (please see attachment), but when I called Chrysler/Dodge help desk and said that I don't have any recall listed in my file. When I called blue driver company they said that information is came from Chrysler. It's says in the recall info (blue driver) that its free of charge, but when I called Chrysler help desk, the dealer will charge me for diagnostics. I just want to know if this recall from blue driver car diagnostic company's recall reports an accurate recall information, this is a safety issue information so that is why I'm concern about it.
