While driving on Sunday night, a car in front of mine hit the brakes requiring me to do the same. At that point, the seat behind my driver's seat disengaged from the locking mechanism and tilted forward. My daughter was in the seat at the time in her carseat. As the seat fell forward her face hit the back of my seat causing an abrasion and black eye. She was taken to her doctor. The van will be taken in to Chrysler for evaluation. Clearly the seat malfunctioned as it was not left unlocked.

Right rear seat recliner assemblies were replaced due to a rattling noise. Sliding doors don't always completely shut, while driving they will open, dealer states this is the design of the door.
