Had the vehicle on the jack and the vehicle fell off the jack, this happened on September 21, 2005. The jack has a small base, it's about 4X5 or 4X6. He contacted the manufacturer, they said to contact NHTSA.
Chevrolet Silverado 2004, inadequate and unsafe bottle jack. The consumer stated the vehicle was too large for a jack that size. The consumer is requesting the manufacturer provide him with an adequate jack.
While placing the jack underneath the vehicle it slipped off. Consumer drove the vehicle to the dealer, and mechanic determined that the jack operated as designed.
