TCM module failure with units with a 150k km (90k mi) is a common issue on other forums. TCM module sits very close to engine with crappy heat insulation. Car will start in safe mode (3rd Gear) and when shifting will be a hard, jolty, grindy shift.
It's incredibly frustrating to drive back to the mechanic. The part alone is expensive from the dealer, and labor required is intensive (hard to reach and replace).
Swapped out at 170k km, now at 183k and no issues. Though I do not know the exact damage done to the transmission while this failure occurred.
